BEFORE YOUR FIRST VISIT
A clearer picture of your costs.
Log into your member portal or call the number on your insurance card. Ask whether Aharon Weinstein at Pathways Counseling & Consultation is in network for your specific behavioral health plan.
- Is outpatient psychotherapy by video covered while I am in Ohio?
- What are my benefits for the intake (CPT 90791) and ongoing psychotherapy (CPT 90837)?
- What deductible remains, and what copay or coinsurance applies?
- Do I need a referral or prior authorization, and are there session limits?
Being in network does not necessarily mean visits are fully covered. Benefits information is an estimate; your plan’s processing of the claim determines your responsibility. See the accepted plans and session fee.

PRACTICAL DETAILS
Fees, payment, and getting started.
What do these insurance terms mean?
Your deductible is the amount you pay toward covered services before your plan starts paying its share. A copay is a fixed amount per visit; coinsurance is a percentage of the allowed cost. Your out-of-pocket maximum is generally the most you pay for covered, in-network services during your plan year. Your specific plan determines how these apply.
When should I check my benefits?
Check before beginning therapy, whenever your insurance changes, and at the start of a new plan year, when deductibles may reset. I cannot see all the details of your individual policy. Your insurer’s estimate is helpful, though the final amount can differ when the claim is processed.
What are the self-pay fee and payment options?
The Pathways self-pay fee is $150 per session. Major credit and debit cards, including HSA/FSA cards, are accepted. Payment is due at the time of service. If cost is a barrier or you prefer self-pay, please reach out to discuss your options.
If you are uninsured or paying out of pocket, you may request a written Good Faith Estimate.
What do I need to do before scheduling?
Complete the intake paperwork you receive and provide clear images of both sides of your insurance card through the intake process before your first session. Once those items are received, I’ll send the next scheduling steps. You’re welcome to ask if any form or insurance term is unclear.
